Liability and Medical Release

I acknowledge that I am medically fit and I have no medical conditions that would interfere with my attendance at the Skills Competition and acknowledge my responsibility to disclose any medical condition that could compromise my safety or the safety of others while I attend or participate in Skills Canada NWT activities. I confirm that I am responsible for my health and acknowledge my responsibility to ensure that I protect myself from any allergies or health concerns that may affect my ability to participate in Skills Canada NWT activities.

I agree to release Skills Canada NWT, its representatives, agents, and employees from liability for any personal injury that may occur while attending any Skills Canada NWT activities, including travel to and from these activities unless the injury or damage results from deliberate acts of Skills Canada NWT representatives, agents, and employees.

I authorize Skills Canada NWT and it’s volunteers to follow necessary emergency medical procedures or treatment using good faith and best judgment. I agree to indemnify and hold harmless Skills Canada NWT for any claims, demands, actions, rights of action, and/or judgments arising from or on account of these emergency medical procedures and/or treatment that were done in good faith and according to accepted medical standards.

2) Photo Release

I agree that any images or videos of me taken during the course of this Skills Canada NWT activity become the property of Skills Canada NWT and may be used and reproduced by Skills Canada and/or its partners in promotional materials, including print, online and social media.

3) Code of Conduct

Skills Canada NWT wants every participant to have an enjoyable experience with maximum attention to safety and comfort. All individuals representing Skills Canada NWT will be expected to behave in a way that reflects well on the
organization.The following “Code of Conduct” has been established by Skills Canada NWT and must be followed at all times.

  • My conduct will be exemplary at all times.

  • I will attend all activities for which I am assigned and will be on time.

  • I will listen and follow all instructions from coaches, advisors and chaperones.

  • I will adhere to the dress code at all times as required.

  • I will spend each night in the accommodation to which I am assigned.

  • I will respect all public and private property, including the accommodation

  • to which I am assigned.

  • I will not use alcohol and/or drugs (except those prescribed by a doctor).

  • I acknowledge that my Advisor/Chaperone is responsible for me from my home

  • departure point and throughout the competition until my return to my original

  • point of departure.

  • I understand that if I violate the Code of Conduct, I may be asked to leave the event or activity and consequently may be excluded from all further Skills Canada NWT events or activities. In addition, I understand that I may responsible for any costs incurred by Skills Canada NWT on my behalf.

By selecting “Yes” within the online registration, you agree to this ‘Code of Conduct' and simply reaffirm your dedication to being the best representative possible. As a participant, you must agree to follow Skills Canada NWT rules and conditions, or you may not be allowed to attend and participate.
