CONSENT AND LIABILITY INFORMATION

General Liability Waiver
I understand and assume all risks associated with participating in the Skills Canada NWT Territorial Competition.
I release and discharge Skills Canada NWT, its employees, volunteers, and affiliates from any claims, damages, or liabilities arising from participation in the competition.
I acknowledge that Skills Canada NWT is not responsible for any personal injury, loss, or damage to personal property that may occur before, during, or after the event.

Media & Photo Release Agreement
I grant permission to Skills Canada NWT to capture and use my name, photographs, videos, and other media content for promotional purposes. This includes, but is not limited to, publications, social media, websites, printed materials, and media broadcasts.
I understand that these materials may be used for an indefinite period unless I revoke this consent in writing.
I waive any rights to financial compensation or approval of the final materials where my likeness appears.
I release Skills Canada NWT from any claims related to the use of these materials.

Medical Authorization
In the event of an emergency, I authorize Skills Canada NWT staff or representatives to seek medical treatment on my behalf if I am unable to do so.
I understand that I am responsible for any medical expenses incurred during the competition.
I agree to inform Skills Canada NWT of any medical conditions, allergies, or required medications prior to the event.

Code of Conduct Agreement
I agree to follow all competition rules and regulations as outlined by Skills Canada NWT.
I will conduct myself in a professional and respectful manner at all times.
I understand that failure to adhere to these rules may result in disqualification.
Any form of misconduct, including cheating, inappropriate behavior, or damage to property, may result in immediate removal from the event.

By registering for the Skills Canada NWT Territorial Competition, I acknowledge that I have read, understood, and agreed to the terms outlined in this consent & waiver package.
